Sample Request From <requester>
In order to protect your anonymity, <requester> will not see which responses came from you. In addition to you, this same request was simultaneously sent to 4 other people. Everyone responds by choosing from the same list of adjectives and <requester> will not see any responses until everyone has responded. If fewer than three people complete the form, all responses will be deleted and you will be notified by email that <requester> will not see any responses.
As long as three or more people respond, <requester> will see each selected adjective along with how many responders selected it. The response summary will be viewable for seven days and then all responses will be permanently deleted. No one will ever see the responses except <requester>.
Please select a minimum of one and a maximum of three adjectives from each column.
